Neglect and 16 points of failure contributed to teenager’s death at care facility, inquest finds

Sophie Bennett was 19 when she took her own life, upset by changes made to the home she lived in, which was run by the charity Richmond Psychosocial Foundation International. These changes included sacking therapists and introducing yoga.

Today an inquest jury found she had not intended to take her own life and that neglect contributed to her death, identifying 16 points of failure in her care.
