Coroner highlights missed opportunities that could have saved teenager Amber Peat

A coroner has said there were at least 11 missed opportunities for agencies to intervene and perhaps save the life of a 13-year-old girl.

Amber Peat was found hanged after walking out of her home in 2015.

The coroner said she could not be sure Amber had intended to take her own life, but in a narrative verdict she raised safeguarding concerns and said professionals had not asked the right questions.
