Babies and mums died amid ‘toxic’ culture at Telford and Shrewsbury hospital, leaked report says

Babies and mothers died. Others were left permanently disabled. Distraught families were left struggling to find out any information from a hospital that repeatedly failed to learn from mistakes.

This could well be the country’s worst ever maternity scandal – as a leaked report into the Shrewsbury and Telford NHS trust found a toxic culture stretching back 40 years.

And, it warns, key lessons are still not being learned there today.
