An inquiry into the death of baby Jessica Randall, killed by her own father, calls for a nationwide programme of child protection training.
Born with a heart defect to a schizophrenic mother, baby Jessica Randall was systematically beaten and sexually abused by her own father.
She was just seven weeks old when Andrew Randall killed her. Today, more then two years later, an inquiry has found that "opportunities had been missed to identify signs of abuse".
The inquiry recommended a nationwide programme of child protection training to improve the protection of such vulnerable children.
