Giuseppe Sculli's late goal broke Napoli's hearts as Genoa came away with the points from a deserted San Paolo.
This match was played behind closed doors after an assistant referee was struck by a plastic bottle thrown from the stands in Napoli's midweek 1-0 victory over Livorno. It was a shame to see a tie with such a glorious history played to empty seats, as they had not met here in Serie A since a 1-0 home win in February 1995 and the two clubs are twinned.
Both were promoted in the summer, but their seasons have been very different so far. Napoli had won three of their last four games, while Genoa finally got off the mark with Marco Borriello's hat-trick - the club's only scorer until now and a Naples homeboy. Alessio Scarpi stepped in for suspended goalkeeper Rubinho.
It was a shaky start for Napoli, as captain Paolo Cannavaro accidentally turned Julio Cesar Leon's cross into his own net in a bid to anticipate Borriello. It was the first goal they had conceded since the Week 1 defeat to Cagliari, ending an unbeaten run of 410 minutes between Serie A and the Coppa Italia.
Genoa tried to take advantage and moments later Matteo Gianello smothered a Borriello rocket, while Cannavaro attempted to make up for his error with a header over the bar.
Alessandro Lucarelli brought down Lavezzi on the very edge of the box, but it was not a red card because Cesare Bovo was there to prevent it being a clear scoring opportunity.
Substitute Marek Hamsik's cross found a spectacular Lavezzi volley just over the bar.
After the restart Lavezzi was sandwiched between Konko and Gaetano De Rosa for a penalty. Maurizio Domizzi buried the spot-kick into the low near corner of the net to draw Napoli level.
Alessio Scarpi palmed a fierce Hamsik rising drive over the bar, then blocked Lavezzi's screamer.
Roberto Sosa came on for the home side, but his header was off target from Marcelo Zalayeta's cross.
Instead, in the final minute Genoa scored against the run of play. Borriello turned into an assist man, his cross finding Giuseppe Sculli's header to come in off the inside of the upright.
