Palermo suffered penalty heartbreak against Mlada Boleslav and exited the UEFA Cup at a deserted Stadio Renzo Barbera.
The Rosanero came into the second leg with a 1-0 lead after Serbian starlet Bosko Jankovic's last-gasp strike in the Czech Republic, but Bolselav gave them a taste of their own medicine with a leveller in the dying seconds.
The Czechs knew they had little right to progress and kept their cool better in the penalty shoot-out to land a scalp equal to that of Marseille last year.
Boss Stefano Colantuono opted for Edison Cavani and Amauri up front and started Fabrizio Miccoli on the bench, knowing that a 0-0 draw would see his men through.
Miccoli had been expected to start in place of Amauri, but he reported a calf pain during the warm-up.
The visitors knew that only a win would suffice and were quick out of the blocks with Jan Kysela firing wide after just a minute.
The Sicilians settled and managed to bring the ball down and slow the game after chasing for the first 10 minutes.
A limp game meandered lifelessly in front of a far from animated crowd. A poor tackle from behind by Leandro Rinaudo on French midfielder Alexander Mendy earned him the first yellow card of the game.
Amauri broke the tedium with a powerful low shot, but it was straight at 'keeper Miroslav Miller and he gathered easily.
The first half ended without any drama and the second period continued in that vein with Palermo happy to keep the tempo slow and Boleslav lacking the flair to force the pace.
It took half an hour for the first shot of the second half as Amauri rose to head, but Miller blocked his effort without any real trouble.
Palermo's best chance of a dire game fell to Cavani, but his effort from inside the box was blocked by a Czech boot.
Just as it seemed Palermo would progress, Boleslav hit them with a sucker punch!
Tomas Sedlacek pounced on a long kick from 'keeper Miller and slotted it past Miller to force extra time that they never looked deserving of.
The air of panic at the Stadio Barbera nearly subsided when Cavani beat Miller with a header, but he was cruelly denied by the woodwork.
A minute before half-time in extra-time, Marek Matejovsky found space on the edge of the Palermo box, but his effort was wide to the relief of the few remaining fans.
As the thoughts turned to penalties, Cavani was fouled on the edge of the box, but the free-kick was wasted due to an infringement in the wall.
With four minutes of added time left Federico Agliardi failed to deal with a routine strike from distance and let the ball strike the post!
It proved to be the last action of the game before penalty kicks.
Radim Holub took first for Boleslav and converted cooly before Cavani's was saved to give the Czechs the advantage.
Matejovsky also scored as did Andrea Barzagli before defender Adrian Rolko blazed over for Boleslav giving Palermo a chance to level.
But the nerves proved too much for Fabio Caserta who also fired too high and Jan Rajnoch's strike went in off the unlucky Agliardi.
Jankovic had to score to keep Palermo in the UEFA Cup and did, but Sedlacek kept cool to win the shoot-out 4-2 and send the Rosanero crashing out of the UEFA Cup in a smash and grab raid.
