Events are examined in terms of the crucial scientific elements that,combined or in sequence, led inexorably to a particular disaster. In some cases, alternative theories have been thrown up and these are discussed in detail. And in most cases, the disasters come with 'silver linings' - changes that have been made in the light of what happened and so have made life safer for those involved in the future.
