The world's worst industrial accident, which took place at Bhopal in India in December 1984, should not have happened. There were warning signs that all was not well: in the four years preceding the accident, six serious accidents had taken place at the plant and one man had lost his life. But thousands more lives were lost because the signs were ignored. Due to a combination of safety failures and poor maintenance, a deadly chemical reaction was allowed to begin. And again, due to inadequate safety measures and faulty equipment, the reaction went undetected until it became unstoppable.
