Matron wants stool samples from everyone and hands out bed pans. The samples are examined by a gastroenterologist. The point Sir Roy Strong is making is that there's a connection between what goes in and what comes out!
The Victorians were obsessed with the bowels and physicians of the period advocated treatments including laxatives and enemas. The Edwardians invented contraptions for purging the bowels and by the 1920s colonic irrigation was in vogue.
At a farmers' market where the dieters are meant to be buying healthy food, Dave sneaks off to a country pub where he sinks a couple of quick pints. When he's found out, Sir Roy is furious and Dave's future at the health farm hangs in the balance.
