Health Secretary Alan Johnston apologises for the unacceptable failures at Stafford Hospital, launching an immediate review.
The NHS watchdog has condemned Stafford Hospital for its "shocking" standards, where emergency care was so appalling patients were put at risk, some dying needlessly.

Families of the victims said the wards were filthy, comparing conditions to the developing world.
