MoD: "serious and systemic failures"

A coroner condemns the MoD over a Hercules crash in Iraq which killed 10 British personnel. Sue Turton reports.
Recording his verdict, he said lack of the special explosion-suppressing foam was a "contributory factor" to the deaths which occurred when the plane was shot down in 2005. The foam has been fitted to the fuel tanks of US Hercules since the 1960s.
He also criticised the MoD's policy of shredding documents which he said plagued the inquest. He accused the MoD of "serious and systemic failures".
