'Mistakes' led to Menezes death

A police marksman tells the inquest into the death of Jean Charles de Menezes that a series of mistakes led to his fatal shooting.
The officer, codenamed C12, told the inquest that he could have stopped the young Brazilian from entering the London tube station where he was killed.
He admitted failing to report that he was close to Mr de Menezes, but said he hadn't heard radio traffic from Scotland Yard.
Mr de Menezes was killed after being mistaken for a suicide bomber.
