The young residents of St Elsewhere work feverishly to keep pace with the flow of patients that courses through the hospital's less-than-adequate facilities.

Presiding over all is the venerated Dr Westphall (Ed Flanders), who is highly respected among residents and nurses. The elitist Dr Craig (William Daniels), a brilliant but tyrannical heart surgeon, continues to find his perfect foil in the insecure, wise-cracking Dr Victor Ehrlich (Ed Begley Jr), who has suffered through more than his share of personal tragedies.

Other characters facing round-the-clock responsibilities include Nurse Rosenthal (Christina Pickles), who struggles to cope with complicated family troubles; liver cancer specialist Dr Auschlander (Norman Lloyd); the irresponsible Dr Wayne Fiscus (Howie Mandel) and Dr Philip Chandler (Denzel Washington).

There's the nervous and clumsy Eliot Axelrod (Stephen Furst); former nurse Carol Novino (Cindy Pickett), now a struggling resident, and self-assured young Dr Seth Griffin (Bruce Greenwood).

Completing the regular cast are Jennifer Savidge as Nurse Lucy Papandrao, Sagan Lewis as Dr Jacqueline Wade, France Nuyen as Dr Paulette Kiem and Bonnie Bartlett as Dr Craig's long-suffering wife Ellen.
