Romano suffers the slight inconvenience of having his arm chopped off by a helicopter rotor blade. It's sewn back on but his days as Chief of Staff are numbered. Newly transferred to Chief of Emergency Medicine - and furious about it - he expresses disdain for the ER and everyone in it, and makes a dramatic decision about his mangled arm.
Gallant and Pratt are at each other's throats, Don Cheadle puts in an appearance as a surgical intern with Parkinson's disease, and Weaver tries to keep her pregnancy a secret whilst confronting an ethical dilemma involving an influential alderman testing positive for syphilis.
Meanwhile, Pratt's in trouble when he's caught with a gun in the ER. As well as keeping a wary eye on his mentally challenged brother, Leon, he also finds time to start - and finish - a relationship with Chen. Corday takes a shine to new surgeon Dr Dorset too.
Kovac's troubled life turns tragic when he gets into a car accident on an icy road. Increasingly self-destructive, and unable to satisfy his sexual appetite, he pays the price for his misdiagnosis that left a patient with brain damage and is forced to consult a psychologist.
After breaking every rule in the book to get a dying Croatian boy to Chicago in a race to save his life, Kovac leaves County General on a relief mission to the war-torn Congo.
Things aren't good for Abby either. She becomes disturbingly close to a mental breakdown and struggles to cope with a multitude of problems, including her missing pilot brother Eric and needy mother, Maggie. Luckily, Carter's on-hand to rescue her - he decides to propose, although later on changes his mind.
After weeks of professional and personal setbacks, Carter leaves the ER - and Abby - to join Kovac in Africa where they risk their lives manning a crude outpost used for a vaccination campaign, and find themselves in the line of fire between warring tribes.
