Karma finally bought Earl his good thing and then ploughed into her at 40 miles an hour! But whilst he and Billie lay in their car accident-induced comas, Earl's mind went to its happy place and did what he trained it to do as a kid – create a beautiful TV world starring him and Billie.

Back in the real world however it turns out Billie, who had run down Earl, had been rundown by ex-Officer Daniels, and because the paramedics chose to rescue the victim with the nicer breasts first, Randy has to steal an ambulance to rescue his brother.

Picking up the gang on the way to the hospital, Randy's trip takes a slight detour after Joy distracts Darnell as he’s wielding the defibrillator, causing Catalina to open the ambulance doors to release Earl's dead soul, and Joy and Earl taking an unexpected trip down the freeway on a wheelie bed.

With an additional roster of wounds on top of the coma, including paddle burns, bugs in his teeth, severely bruised nipples and an involuntary orgasm, Earl finally makes it to hospital. But whilst the doctors can save his magnificent moustache, can they save the rest of his body?
